Zero zombie reminders

Cancel the visit, cancel the reminder.

When an appointment is cancelled, every reminder still sitting in the queue for it is killed in the same sweep. When one is moved, the queued reminders are retimed against the new slot. This is not a horizon-limited job that only looks a day ahead — it reconciles the whole queue.
